Putting a duvet cover on is one of those jobs that feels like it should take two minutes and somehow takes fifteen, usually ending with you inside the cover wondering where the corners went.
There is a method that removes the struggle entirely. It is called the roll method, and once you have done it twice you will not go back to wrestling.
The Roll Method, Step by Step
1. Turn the cover inside out and lay it flat on the bed, opening at the foot end.
2. Lay the duvet insert on top of the cover, lining up all four corners as closely as you can.
3. Tie or clip the two top corners. If your cover has corner ties, use them now. If not, pinch each corner of the insert together with the matching corner of the cover and hold.
4. Roll both together from the top down, like a sleeping bag or a long swiss roll, until you reach the opening at the foot.
5. Reach into the opening and pull the cover around the ends of the roll, turning it right side out over the rolled bundle.
6. Unroll. The cover comes with it, already in place.
7. Shake and close. Give it one shake to settle the insert, then fasten the buttons or ties.
The whole thing takes about two minutes once it is familiar. No climbing inside required.
The Inside-Out Method
The alternative, useful if you are on your own with a King size and no floor space.
Turn the cover inside out, reach through the opening and up to the two far corners, and grab them from the inside so your hands are effectively in the cover's corners. Now grip the two top corners of the insert through the fabric. Shake the cover down over the insert while holding on.
It works, but it takes more arm strength and more shaking than the roll method. For anything larger than a Full, roll it.
Why It Goes Wrong
Almost every failed attempt comes down to one of these.
Wrong size insert. An insert much smaller than the cover slides around and bunches at one end. One much larger will not sit flat and strains the seams. Match the sizes.
Ignoring the corner ties. Most quality covers have interior ties or loops at the corners. They exist precisely to stop the insert migrating. Using them takes thirty seconds and solves the problem permanently.
Starting from the wrong end. The opening should be at the foot of the bed while you work. Starting from the closed end means you have nowhere to pull the cover through.
Doing it on a small surface. A bed gives you room to lay everything flat. A sofa does not.
Getting the Size Right
The insert and cover should be the same size. If you are between sizes, it is better for the insert to be very slightly smaller than the cover, so it sits inside without straining the seams.

Keeping the Insert From Shifting
Use the corner ties. That is genuinely most of the answer.
If your insert has no loops to tie to, a few safety pins at the corners work, or you can sew small ribbon loops onto the insert once and never think about it again.
Shake the duvet out each morning rather than only when it bunches. Thirty seconds of redistribution stops the filling from migrating and settling unevenly over time.
How Often to Wash It
Every one to two weeks is a reasonable rhythm for most people, more often in summer or if you sleep without a flat sheet underneath.
This is the practical case for using a cover at all: the cover washes easily, the insert does not. If you sleep directly against a bare insert, it takes all the wear and cannot be laundered nearly as often.
Care
Wash the cover alone the first time. Use a gentle cycle at 86°F (30°C) and tumble dry on low heat. If air drying, support the cover evenly in a well ventilated area and avoid stretching the fabric while wet.
Do not use bleach and do not dry clean. Avoid washing with jeans or anything that has small hooks, which can catch the fabric. Muslin needs no ironing and softens with every wash.
Close the buttons or ties before washing. An open cover in a machine turns into a bag and collects everything else in the load.
